LA’S ACCREDITATION CRITERIA

IN ACCREDITING ACCORDING TO LST EN ISO/IEC 17025

1. Personnel (p. 5.2)

1.1. Requirements to the personnel, evaluating the test results and approving the calibration certificates and test reports:

1.1.1 Education: university or equivalent technical or higher education conforming to the field of activity.

1.1.2 Experience: not less than 3 years in the relevant field of testing and calibration in the last period.

1.2. Requirements to quality managers:

1.2.1 Education: university or equivalent technical or another conforming to the field of activity.

1.2.2 Experience: not less than 1 year in the relevant field of testing and calibration in the last period.

1.2.3 Training: training in quality management.

1.3. Requirements indicated in p.1.1 and 1.2 shall be applied also to the substituting persons.

1.4. Requirements to the personnel of non-destructive testing:

1.4.1 Personnel performing non-destructive methods have to be certified according to LST EN ISO 9712 and depending on complexity of the tasks performed shall have II or III qualification level for the relevant method;

1.4.2 Personnel evaluating the test reports shall be certified according to LST EN ISO 9712 and shall have not less than II qualification level for the relevant method;

1.4.3 Personnel approving the test reports shall be certified according to LST EN ISO 9712 and shall have not less than III qualification level for the relevant method;

1.4.4 Personnel approving the test reports temporary can be substituted by the persons having not less than III qualification level and experience no less than3 years for the radiography and ultrasonic method.

1.4.5 The person responsible for nondestructive testing shall be responsible for following activities:

-authorization of NTD personnel as competent to perform specific inspections/tests and/or to release results;

- approving test procedures and validating methods;

Note: according to EN ISO 9712 for authorization of test procedure is level 3. For routine test procedures according to testing standards, sufficient is working NTD instruction approved by persons of level 2.

1.5. In the documents of the management system of the laboratory defining requirements to personnel, qualification requirements have to be defined, among them – min experience in the relevant field of activity.

2. Equipment (p. 5.5)

2.1. Calibration laboratories shall have all reference standards, equipment and measurement instruments needed for performance of all calibrations indicated in the scope of accreditation.

2.2. In exceptional and motivated cases testing laboratories can use hired equipment and measurement instruments but it can compose just very inconsiderable part of the equipment and measurement instruments needed for the performance of the tests indicated in the scope of accreditation.

2.2.1. Accredited testing laboratory shall have the procedure in which has to be indicated how the maintenance of the hired equipment or measurement instrument also it’s measurement traceability to the national or international standards will be ensured; how it will be ensured that work with the equipment or measurement instrument conforms to the requirements of the standard.

2.2.2. The decision on accreditation of the testing laboratories using the hired equipment or measurement instruments in each particular case is taken by the Lithuanian National Accreditation Bureau.

3. Opinions and interpretations (p. 5.10.5)

If the laboratory decides to present opinions and interpretations, it shall:

3.1. To define the range of the accredited activity within which options and interpretations will be presented.

3.2. To define the requirements (qualification, experience, etc.) to the personnel presenting the opinions and interpretations.

Requirements to these personnel have to be not less than indicated in the p. 1.1 and p.1.4 of this document and taking into account the note 2 of p.5.2 of LST EN ISO/IEC 17025.

3.3. To appoint such persons.

3.4. To define the format of the presentation of opinions and interpretations.

4. Organization of the activity of laboratory (p. 4.1.3)

If laboratory is located in several sites:

4.1 Each site has to be identified and accredited only for those tests or calibrations which can be performed using the equipment being at this site;

4.2 It can be established and identified mobile laboratories, related with the main site of the laboratory, having the personnel and provided with all equipment needed for performance of tests and calibrations foreseen (according to the purpose of the mobile laboratory);

4.3 It can be established and identified mobile laboratories, related with the main site of the laboratory, having the personnel and provided with all equipment needed for performance of tests and calibrations foreseen (according to the purpose of the mobile laboratory).
